Abstract


Psoriasis is an inflammatory autoimmune disease that affects the skin. Recently, psoriasis and its consequential lifestyle and dietary habits have been associated with increased risks for cardiovascular diseases. This article discusses the connection between cardiovascular disorders and psoriasis and the effects of available treatment options on cardiovascular risk. A PubMed search revealed 11 articles that were analyzed for information regarding this association, its effects, and potential courses of treatment. Both the presence and severity of psoriasis increases the risk for cardiovascular disorders and co-morbidities. Forty percent of psoriasis patients met metabolic syndrome criteria as compared with 23 % of non-psoriasis control subjects. Rate ratios for atrial fibrillation are correlated with the severity of psoriasis; patients with severe and mild psoriasis produced rate ratios of 1.63 and 1.31, respectively. Studies also show an increase in the risks for myocardial infarction, atherosclerosis, ischemic stroke, and other cardiovascular disorders. The exact mechanisms behind this affiliation are still uncertain; however, the psychological and physiological effects of psoriasis and the overlapping pathogenesis behind atherosclerosis and psoriasis may play a role. Since the risk for cardiovascular disorders increases with the presence and severity of psoriasis, psoriasis treatment should not only address the disease and its symptoms, but also its co-morbidities. Recent National Psoriasis Foundation (NPF) guidelines have provided recommendations for psoriasis patient care. Histories of co-morbidities, screenings for potential diseases, increased exercise, decreased alcohol consumption, and smoking cessation should be implemented. Unfortunately, while there are data for the increased risk for cardiovascular diseases within psoriasis patients, there are presently no data stating that increasing cardiovascular screening rates in patients produces a significant difference.

Abstract


Lymphedema is a progressive condition that can have a marked physical and psychological impact on affected patients and significantly reduce the quality of life. The ulcers on chronic lymphoedema patient, which often also makes it impossible for them to work. If left untreated, tends to progress or worsen. Ulcers in lymphoedema patients, therefore, represent not only a medical but also a psychological problem. The treatment is often regarded as being worse than it actually is. In our study of more than 25 years shows around 10% cases are due to chronic lymphodema. Ulcers of chronic lymphoedema are classified into four stages according to their presentation. Their management depends upon their stage of presentation. Patients with chronic lymphoedema and ulceration require a different approach to treatment. The specific issues associated with managing the patient with lymphoedematous ulceration include, limb shape distortion i.e., elephantiasis, care of the skin creases and folds, and swelling of the toes and fore foot. Stage I ulcers will heal with conservative treatment without any surgical intervention. Stage II ulcers needs debridement of the wound and split-thickness skin grafting. The most difficult to treat are the stage III and IV ulcers, due to associated skin changes and reduced vascularity. These cases need debulking along with excision of the ulcer. In order to prevent recurrence of the ulcer in all the four stages needs prolonged follow-up and limb care.

Abstract


Lymphedema is a chronic, progressive, and often debilitating condition. Primary lymphedema is a lymphatic malformation developing during the later stage of lymph angiogenesis. Secondary lymphedema is the result of obstruction or disruption of the lymphatic system, which can occur as a consequence of tumors, surgery, trauma, infection, inflammation, and radiation therapy. Here, we report a 64-year-old woman presenting with hyperkeratosis, a lymphedema due to metastatic uterus carcinoma. In this paper, we present the effects of complex decongestive physiotherapy on lymphedema and hyperkeratosis.

Definition:
A chronic skin disease that presents with characterists of malignant T-cell lymphoma. However, it is important to remember that lymphotoid papulosis (LyP) is not classified as a true lymphoma. Also, it does not spread and is not fatal. It is therefore described as histologically malignant, but clinically benign. (1)
It is classified as a lymphoproliferative disorder. These account for about 25% of cutaneous T-cell lymphomas. The condition occurs equally between men and women and usually occurs in the fifth decade. Clinically, black people appear to have the disease much less frequently then other races.
The term lymphomatoid papulosis originally was used by Macaulay[1] in 1968 to describe “a self-healing rhythmical paradoxical eruption, histologically malignant but clinically benign.” Due to the typical waxing and waning clinical course, lymphomatoid papulosis was previously considered a pseuodolymphomatous inflammatory process. However, the classification system for cutaneous lymphomas has evolved rapidly, and, during consensus meetings in 2003-2004, the World Health Organization—European Organization for Research and Treatment of Cancer (WHO-EORTC) classification grouped lymphomatoid papulosis among the indolent cutaneous T-cell lymphomas. LyP is divided into three subtypes, they include:
Type A - characterized by large CD30 atypical cells intermingled with a prominent inflammatory infiltrate. The large tumor cells have polymorphic convoluted nuclei with a minimum of 1 prominent nucleolus and resemble Reed-Sternberg cells when binucleate, as is seen in HD. Type A lymphomatoid papulosis is the most common histologic variant and accounts for 75% of all lymphomatoid papulosis specimens.
Type B is characterized by smaller (8-15 µm) atypical cells with hyperchromatic cerebriform nuclei resembling the atypical lymphocytes in MF. CD30+ large cells are rare, but epidermotropism is more common in this variant. There is some concern that Type B lymphomatoid papulosis may be better classified as a papular variant of MF.
Type C (diffuse large cell type) is characterized by sheets of CD30+ anaplastic large cells indistinguishable from ALCL, with the exception of the minimal subcutaneous invasion. These lesions resolve spontaneously and are therefore classified as lymphomatoid papulosis; however, some authorities view this histologic variant as borderline ALCL or, perhaps, pcALCL. (2)
Uncommonly, patients may have more than one histologic subtype of lymphomatoid papulosis or other recently described associated histologic patterns.
Risk Factors:
Unknown at the present time and there is debate on whether or not it may be genetically caused. Several genetic defect have been identified in LyP lesions, but the specific cause has not been identified.
A few investigators have also discovered viruslike particles in lymphomatoid papulosis lesions examined under electron microscopy.
Signs and Symptoms:
Lymphomatoid papulosis appears as recurrent small, raise skin lesions. The color ranges from red to brown in color. The most commonly occur on the trunk, arms, and legs in crops but may also occur on the palms, soles, face, and scalp. Very rarely, LyP lesions may be present in the mouth and throat. The lesions heal spontaneously within several weeks and leave a small, hypopigmented scar.
Diagnosis:
Done by skin biopsy. This is critical as the lesions may appear as the same for numerous other skin nodular growths.
Biopsies are also critical to rule out mycoses fungoides, cutaneous anaplastic large cell lymphoma, cutaneous Hodgkin’s disease, cutaneous leukemia, scabies, insect bites, and drug reactions.
If after biopsy the result come back other then lymphotoid papulosis, then the treating physician will order other types of tests to establish a correct diagnosis.
Differential Diagnosis
Cutaneous CD30+ (Ki-1) Anaplastic Large-Cell Lymphoma, Cutaneous T-Cell Lymphoma, Folliculitis, Insect Bites, Langerhans Cell Histiocytosis, Leukemia Cutis, Lymphocytoma Cutis, Milia, Miliaria, Scabies
Prognosis:
The disease itself is not fatal. However, 10 to 20% of patients will also develop an associated systemic lymphoma, typically anaplastic large cell, Hodgkinís disease, or mycoses fungoides.
If any type of secondary malignancy develops, then that would alter the outlook, depending on the type of malignancy.
Treatment:
Patients may choose not to treat the lesions and they usually heal spontaneously over 1-2 months.
Historically, treatment for the skin lesions themselves has been corticosteroid creams or ointment. Another option that can lead to faster healing is with low dose methotrexate. This is a type of chemotherapy that inhibits cell division. The one weak point in this treatment is that the lesions treated with methotrexate will generally return several weeks after therapy.
Another strong, aggressive treatment is oral psoralen plus ultraviolet light, so-called PUVA therapy.
Other treatments might include: carmustine, topical nitrogen mustard, topical MTX, topical imiquimod cream, intralesional interferon, low-dose cyclophosphamide, chlorambucil, medium-dose UVA-1 therapy, excimer laser therapy, photodynamic therapy, and dapsone help disease suppression.
Mortality/Morbidity
Lymphomatoid papulosis has a chronic, indolent course in most patients.
However, associated lymphomas may arise with LyP. These include immunoblastic lymphoma, lethal midline granuloma (currently considered as natural killer cell lymphoma in many patients), and systemic lymphocytic lymphoma. In most patients, the malignancy develops many years after the diagnosis of lymphomatoid papulosis.

These are small usually round nodules that seem to literally pop out of the skin on late stage lymphedema patients.  Normally, they are skin colored, can grow singularly or in clusters.  For the population at large, these are almost always benign growths that really require no followup or special care.

However, in lymphedema patients these can become angiosarcoma and thus need to be promptly removed with the nodules sent to pathology lab for examination.  

I have had many removed over the years and in fact just recently had well over twenty removed from the knee areas of both legs.  The surgeon "snips" the stem that these grow on and removes them.  For the most part, they can be removed on an outpatient basis.  There are no stitches involved either and after healing there is practically no scar either.

The dressing is simple.  I used an antibiotic ointment covered with a smooth bandage to go directly over the wound, then a layer of gauze.  Since my skin will not tolerate any type of tape, I keep it in place using my short stretch bandage on the leg.  Don't put gauze directly onto the wound as it will get stuck in the seepage of the wound and can cause damage to the wounds healing processes.

I would also recommend an antibiotic to be taken as a preventative measure to help insure there is no infection or cellulitis due to the immunocompromised state of a lymphedematous limb.

Abstract 


Stasis papillomatosis can be divided into localized and widespread forms. Ten patients with this disease are discussed with emphasis on their clinical and lymphoradiological findings. Using isotope lymphography we were able to find an overall lymphostasis in only four patients. In all patients, local lymphatic disturbances were detected by means of indirect lymphography with intradermal lymph cysts as the most specific sign. Local dermal lymphostasis seems to be the common final pathogenesis in spite of different etiologies (ie, primary lymphedema, chronic venous insufficiency, trauma recurrent erysipelas, and local lymphangiodysplasia of unknown origin). A maximum variant was seen, following en-bloc resection of subcutaneous tissue in a patient suffering from congenital lymphedema.

Because of the compromised condition of a lymphovenous limb, we are often susceptible to a large number of skin complications.  These may include various skin growths such as skin tags, warts, dermatofibromaslymphangiomas,and rashes, fungal infectionsThis is in addition to the many infections associated with lymphedema  superficial bacterial infections which include as impetigofolliculitiscarbuncles, furuncles and boils and weeping wounds or sores.

With lymphedema, some types untreated skin conditions can lead to serious consequences including systemic infections (sepsis), gangrene, amputation and even death.  Good skin care and health is critical to our overall good health.


Skin Growths


Skin growths are accumulations of various types of cells that look different than the surrounding skin. They may be raised or flat and range in color from dark brown or black to flesh-colored to red. Skin growths may be present at birth or develop later.



When the growth is controlled and the cells do not spread to other parts of the body, the skin growth (tumor) is noncancerous (benign). When the growth is uncontrolled, the tumor is cancerous (malignant), and the cells invade normal tissue and even spread (metastasize) to other parts of the body. Noncancerous skin growths are often more of a cosmetic problem than anything else.


Doctors do not know what causes most noncancerous skin growths. Some growths, however, are known to be caused by viruses (for example, warts), systemic (bodywide) disease (for example, xanthelasmas or xanthomas caused by excess fats in the blood), and environmental factors (for example, moles and epidermal cysts stimulated by sunlight).

Dermatofibromas 

Dermatofibromas are small red-to-brown bumps (nodules) that result from an accumulation of collagen, which is a protein made by the cells (fibroblasts) that populate the soft tissue under the skin.

Dermatofibromas are common and usually appear as single firm bumps, often on the legs, particularly in women. Some people develop many dermatofibromas. 
Causes include trauma, insect bites, and cuts caused by shaving. Dermatofibromas are harmless and usually do not cause any symptoms, except for occasional itching. Usually, dermatofibromas are not treated unless they become bothersome or enlarge. A doctor can remove them with a scalpel.

Epidermal cysts

An epidermal cyst is a common slow-growing bump consisting of a thin sac of skinlike material containing a cheesy substance composed of skin secretions.


Epidermal cysts, often incorrectly referred to as sebaceous cysts, are flesh-colored and range from ½ to 2 inches across. They can appear anywhere but are most common on the scalp, back, and face. They tend to be firm and easy to move within the skin. Epidermal cysts are not painful unless they become infected or inflamed.

Large epidermal cysts are removed surgically after an anesthetic is injected to numb the area. The thin sac wall must be removed completely or the cyst will grow back. Small cysts may be injected with corticosteroids if they become inflamed. Infected cysts are treated with an antibiotic and cut open to drain. Tiny cysts that are bothersome can be burned out with an electric needle.


Because sunlight may stimulate growth of epidermal cysts, fair-skinned people are advised to stay out of the sun and to use protective clothing and sunscreen.

Dermoid cyst

A benign tumor which is made up of hairs, sweat glands, and sebaceous glands. Some internal dermoid tumors may even contain cartilage, bone fragments, and teeth. Dermoid cysts may be removed surgically for cosmetic reasons.

Keloids

Smooth, firm, raised, fibrous growths on the skin that form in wound sites. Keloids are more common in African-Americans.

Keloids respond poorly to most treatment approaches. Injections of corticosteroid drugs may help to flatten the keloids. Other treatment approaches may include surgery or silicone patches to further flatten the keloids.

Keratoacanthomas

Round, flesh-colored growths that have a crater that contains a pasty material. These growths tend to appear on the face, forearm, or back of the hand. They usually disappear after a couple of months, but may leave scars.
Treatment usually includes a skin biopsy to rule out skin cancer. Other treatment may include surgical removal and/or injections of corticosteroids or fluorouracil.

Lipomas

Round or oval lumps under the skin caused by fatty deposits. Lipomas are more common in women and tend to appear on the forearms, torso, and back of the neck.
Lipomas are generally harmless, but if the lipoma changes shape, your physician may perform a biopsy. Treatment may include removal by surgery, if the lipoma bothers the child.

Pyogenic granulomas

Red, brown, or bluish-black, raised marks caused by excessive growth of capillaries (small blood vessels) and swelling. Pyogenic granulomas usually form after an injury to the skin.

Some pyogenic granulomas disappear without treatment. Sometimes a biopsy is necessary to rule out cancer. Treatment may include surgical removal.

Seborrheic keratoses

Flesh-colored, brown, or black wart-like spots. More common in middle-aged and older people, seborrheic keratoses may be round or oval and look like they are "stuck" on the skin.

Usually, no treatment is necessary. If the spots are irritated, or the patient wants them removed for cosmetic reasons, treatment may include freezing the area with liquid nitrogen or surgery.

Abstract


Background:Obesity is one of the world's biggest health problems nowadays. Little research has been done on the skindiseases that affect obese patients. 

Objective: To study the prevalence of skin manifestations in obese patients compared with a control group of normal-weight patients. 

Methods:A total of 76 obese patients [body mass index (BMI) ≥30 kg/m(2) ] and 73 with normal-weight volunteers (BMI 18.5-24.9 kg/m(2) ) were included in the study and had their complete medical history and skin examination evaluated by the same examiner. All patients were investigated for the presence of metabolic syndrome. 

Results:The dermatoses that showed a statistically significant relationship with obesity, compared with the control group were: striae (P < 0.001), plantar hyperkeratosis (P < 0.001), acrochordons (P = 0.007), intertrigo (P < 0.001), pseudoacanthosis nigricans (P < 0.001), keratosis pilaris (P = 0.006), lymphedema (P = 0.002) and bacterial infections (P = 0.05). The presence of striae, pseudoacanthosis nigricans and bacterial infections were also found to be correlated with the degree of obesity. 

Conclusions:Obesity is strongly related to several skin alterations that could be considered as markers of excessive weight. Skin care of obese patients deserves particular attention, not only because of the high prevalence of cutaneous alteration but mainly because many of these disorders are preventable and could be treated, improving patient's quality of life.

Abstract


PURPOSE:

The sequelae for patients living with stage III lymphedema, including immobility, functional impairment, isolation and emotional burden, that affect quality of life are highlighted.

METHOD:

The case patient's medical history upon initial evaluation, descriptive wound characteristics and treatment plan are described in detail.

RESULTS:

A comprehensive treatment regimen resolved the patient's wound.

DISCUSSION:

The definition, staging and prevalence of lymphedema are reviewed.

CONCLUSION:

Successful lymphedema wound treatment includes working with experienced healthcare providers, management of wound drainage and odor, the use of higher pressure compression, and complying with lifestyle changes.

However, if a patient had surgery involving the lymphatic system – a radical mastectomy for example – even many years ago, the risk for developing lymphedema will always be present, even if this patient never had any arm swelling.

The skin is the first line of defense against foreign invaders and is usually impermeable to bacteria and other pathogens. However, any defect in the skin such as burns, chafing, dryness, cuticle injury, cracks, cuts, splinters, and insect bites can present an entry site for pathogens or infectious agents and cause infection.

Current methods of cancer treatment lower the risk of developing lymphedema. However, if a patient had surgery involving the lymphatic system – a radical mastectomy for example – even many years ago, the risk for developing lymphedema will always be present, even if this patient never had any arm swelling.

Prevention is in the patients’ best interest and a great tool to avoid the onset of lymphedema. The skin is the first line of defense against foreign invaders and is usually impermeable to bacteria and other pathogens. However, any defect in the skin such as burns, chafing, dryness, cuticle injury, cracks, cuts, splinters, and insect bites can present an entry site for pathogens or infectious agents and cause infection.

Patients who already have, or had lymphedema are susceptible to infections of the skin and nails. Lymphedematous tissues are saturated with protein-rich fluid, which serves as an ideal nutrient source for bacteria and other pathogens. Lymphedematous skin also tends to be dry and may become thickened and scaly, which increases the risk of skin cracks and fissures.

The process of inflammation may not only make lymphedema much worse by increasing the swelling, but can also develop into a serious medical crisis. The basic consideration in skin and nail care is therefore the prevention and control of infections, which includes proper cleansing and moisturizing techniques with the goal of maintaining the health and integrity of the skin.

Suitable ointments or lotions formulated for sensitive skin, radiation dermatitis and lymphedema should be applied before the application of lymphedema bandages while the patient is in the decongestive phase of the treatment. Once the limb is decongested and the patient wears compression garments, moisturizing ointments should be applied twice daily.

Ointments, as well as soaps or other skin cleansers used in lymphedema management, should have good moisturizing qualities, contain no fragrances, be hypo-allergenic and should be in either the neutral or acidic range of the pH scale (around pH 5). To identify possible allergic reactions to skin care products, they should be first tested on healthy skin before the initial application to areas affected by lymphedema.

Tight fitting compression sleeves or stockings, as well as materials used in compression bandaging may also cause skin irritation. Some patients may be allergic to a certain material used for compression therapy. This situation can be remedied by switching to other materials.

In mosquito infected areas it is necessary to apply insect repellents to the affected extremity (some moisturizers contain natural repellents) to avoid bites, which are a common cause for infections.

Lymphedema Skin Care Information

We should daily, before wrapping or bandaging,use skin lotion to help keep the skin soft and to prevent chapping or cracking from dryness. Our skin is the front line of defense against invasive bacterial or fungal infections.
